With the Baker County Property Appraiser, and the deadline is March 1 for the tax year you are claiming. That holds for a house inside Macclenny as well: the city levies its own tax, but the exemption application still goes to the county Property Appraiser. Use the parcel number from your closing file so the application matches the record.

Can I split Baker County acreage to put a second home on it for a relative?

Do not buy on that assumption.
